Heat-Related Critical Illness and ICU Preparedness

Heat stroke requires fast recognition

In the emergency department, the first challenge is identifying which patients are at risk of deterioration. A patient who appears dehydrated may also have evolving organ dysfunction. A patient with altered mental status may require aggressive cooling, airway monitoring, and ICU-level care. A patient with exertional heat illness may also develop rhabdomyolysis, electrolyte abnormalities, or acute kidney injury.
These cases often require coordination across ED, ICU, nursing, laboratory, pharmacy, and respiratory therapy teams.
AKI and rhabdomyolysis can complicate the picture
Heat-related illness does not end with temperature control. Patients may develop acute kidney injury due to dehydration, hypotension, muscle breakdown, or multi-organ stress. Rhabdomyolysis can also occur, particularly after exertion, prolonged immobilization, or severe heat exposure.
Because symptoms can overlap with dehydration or general heat illness, testing and monitoring are critical. Patients may require fluid management, renal monitoring, electrolyte correction, and ICU-level observation depending on severity.
For hospitals without immediate access to nephrology, intensivists, or specialty consultation, these cases can create difficult decisions about monitoring, escalation, and transfer.
Where breakdowns can happen
Heat-related critical illness can expose weaknesses in hospital workflows. From an operational standpoint, risk increases when clinical deterioration outpaces the hospital’s ability to escalate care, allocate staff, or coordinate the right level of support.
Breakdowns may occur when:
- Heat illness is not recognized early
- Patients are monitored in lower-acuity settings despite risk of deterioration
- ED-to-ICU escalation is delayed
- Staffing is stretched during high-volume heat events
- Specialty support is not readily available
- Transfer decisions are delayed until the patient is unstable
These are not simply clinical challenges. They are operational challenges that require planning, communication, and consistent decision-making.
Why ICU readiness matters before the heat wave
Hospitals should prepare for heat-related critical illness before extreme heat events occur. This includes reviewing protocols, clarifying escalation criteria, ensuring cooling resources are available, and confirming that teams understand when critical care support should be involved.
Preparedness should also account for after-hours coverage. Heat-related emergencies do not arrive on a predictable schedule. Hospitals may need critical care input at night, on weekends, or during periods when local staffing is already limited.
